East Lake Street corridor residents, business owners, visitors and stakeholders are invited to use this online form to refer individuals who are engaging in non-violent law violations to LEAD Minneapolis. See map.

Common examples of the issues that LEAD is meant to address include, but are not limited to, shoplifting, drug use in bathrooms, and loitering at transit stations. If you’re unsure, we welcome your referral regardless. You can also reach us by phone for more information:

  • Who is eligible?

    In order to meet the criteria, an individual must:

    • have a known or presumed history of or be at risk for repeated contact with the criminal legal system due to underlying substance use, mental health illnesses, homelessness, or extreme poverty
    • be over the age of 18
    • reside in or be a frequent visitor of our East Lake Street corridor geographic catchment area from Chicago Avenue to Hiawatha Avenue AND two blocks north and two blocks south:

  • What does LEAD do with this information? Who has access to it?

    The details of your referral will be shared with a LEAD Minneapolis Manager who will determine if the referral is LEAD eligible. If your referral to LEAD is accepted, our Case Managers will attempt to outreach and engage the referred individual.

    One of LEAD’s Golden Rules is that no participant can be worse off because of the sharing of information. The information you share will go directly to LEAD – not police or prosecutors.

  • Will I receive an update on the outcome of my referral?

    When you make a referral, you can choose whether or not you want to receive follow-up information. If you opt-in to receiving follow-up through our referral form, a member of LEAD’s staff will be in contact with you to:

    • Collect additional information needed about your referral to aid case manager outreach (if needed), and
    • Communicate to you general information about whether your referral was accepted. We take confidentiality and privacy seriously. We cannot share personal details with you about the person that you referred.
